Children who have quarantine measures ordered are issued with a CCR by the attending physician.
For children who do not have quarantine measures ordered and cannot attend school due to the interruption in service, the CSSSZ has issued a methodology and new form.

You will claim for the previous calendar month (for example, at the beginning of November for the month of October).
After completing and printing the form, you hand it to your employer.
Be sure to note the application number generated. ("This number will then be used to easily identify the batch case and match documents for batch processing. It will need to be completed on the next calendar month's form if the emergency measures and related school closures continue uninterrupted. Therefore, parents are asked to make careful note of this form number or save the claim form with the number on their computer.")
